Overview

The Grant Position Verde Valley Parent Partnership Worker 2 (GPPW), or Family Support Specialist under the direct leadership of the Manager/Supervisor, is responsible for providing home-based parenting information and education, developmental assessments, parenting assessments, plan development, emotional support, and preservation of the medical home for at-risk families.

Responsibilities

Administrative
* Provides referrals and resources based on family needs.

* Assists Supervision in monthly billing documentation and data collection.

* Responsible for performing a variety of clerical duties, including collection and data entry for the home visiting program.

* Follows up with referrals and identified issues.
 
Community Outreach
* Serves as a liaison for the community by for building, working with and establishing relationships.

* Assists with development of community support and education groups.
 
Operations
* Demonstrates focus placement on strengthening parent-child relationship (improving parenting skills, reducing family stress, and meeting the bio-psychosocial needs of infants, children and their parents).

* Coordinates play groups.

* Demonstrates advanced training and skills in development, children with special needs, family retention, advanced substance abuse, and family violence.

* Performs initial assessment and evaluation to potential, eligible families.

* Coordinates and administers daily referrals and screenings of patients for admittance into the program.

* Administers the Parent Survey (Family Stress Checklist) with all families who volunteer to participate in the needs assessment process.

* Collaborates with Program Manager/Supervisor to discuss and review each referral.

* Assists the Supervisor in client satisfaction phone calls, and completes appropriate documents.

* Assists the Supervisor in completing home observations for staff, and completion of documents.

* Demonstrated proficiency of initial assessment and evaluation of potential, eligible families.

* Creates safe, trusting atmosphere with families.

* Ensures appropriate curriculum is based on individual family needs.

* Maintains positive community relationships.

* Develops and maintains family plan of care progression and/or accomplishments by families.
 
Staff Development
* Assists Supervisor with staff development.

* Assists the Supervisor in participation in orientation of new staff.

* Assists Supervisor win scheduling education opportunities for staff.
 
Compliance/Safety
* Responsible for reporting any safety-related incident in a timely fashion through the Midas/RDE tool; attends all safety-related training programs; performs work in a safe manner; monitors work environment for possible safety issues and ensures others are also performing work in a safe manner.

* Stays current and complies with state and federal regulations/statutes and company policies that impact the employee's area of responsibility.

* If required for the position, ensures all certifications and/or licenses are up-to-date and valid prior to expiration dates.

* Completes all company mandatory modules and required job-specific training in the specified time frame.

Qualifications

Education
Associate's Degree in Nursing, Social Work, or health related field- Required
Bachelor's Degree- Preferred
 
Certification & Licensures
BLS (American Heart Association)- Required
Current Arizona drivers license- Required
Fingerprint Clearance Card application number- Required upon hire
Fingerprint Clearance Card- Required within 90 days of hire
 
Experience
Minimum 3 years child development experience- Required
Previous computer/word processing experience- Required
Experience with home visiting programs- Preferred
Bilingual- Preferred

Healthcare is a rapidly changing environment and technology is integrated into almost all aspects of patient care. Computers and other electronic devices are utilized across the organization and throughout each department. Colleagues must have an understanding of computers, and competence in using computers and basic software programs.
