The LDRP RN Case Coordinator functions as a registered nurse and case coordinator of patient care for the Labor and Delivery Post Partum Unit patients and their families including patient and family education, assessment, planning, communication and evaluation of services to address their needs in order to achieve safe and efficient patient family centered care. While this role does not provide direct patient care, the role follows the nursing process of assessing, nursing diagnosing, outcome identification, planning, implementing and evaluation in relationship and collaboration with the Clinic's multi-disciplinary team for the delivery of care at the Labor and Delivery Post Partum Unit. The Registered Nurse serves as a liaison within the LDRP Unit for ancillary personnel and providers by being an active member of the healthcare team. This position practices within the ANA Scope of Practice, the ANA Code of Ethics for Nurses and the standards of his/ or her specialty, as applicable. |
Profile RN * Schedules patients for a phone interview to enter demographic information, health history and prenatal care. * Provides patient education on what to expect on LDRP unit as a patient, labor, delivery, postpartum, lactation and pre-op teaching as well as community resource information. * Performs insurance verification, creation of FIN, collecting and verifying patient history, medications and issues. * Verifies Prenatal records are present on LDRP Unit or obtains PNR from OB Provider, enters pertinent data from PNR in the patient’s EMR. * Verifies Scheduled cases have Order Sets placed by OB Provider in planned state on patient’s EMR. * Identifies patient’s with High Risks and organizes a Predelivery Multidisciplinary Care Conferences as need by scheduling, planning and facilitating the Care Conference. * Schedules and conducts Patient-Family Tour of LDRP Unit per established guidelines. * Reviews and updates LDRP Patient Portal with important LDRP Unit information for patients. |
Patient Care * Assesses the needs and conditions of assigned patients by gathering relevant patient health information. * Quickly establishes trusted rapport with patients and families over the phone to promote quality assessment and education. * Quickly solves problems using critical thinking skills, responds to phone emergencies and other patient situations in a timely and professional manner. *Identifies outcomes, develops a plan of care individualized to the patient and/or situation, and implements the identified plan appropriate for the attainment of safe and efficient patient-family centered care. * Contributes to patient experience by liaising with Provider Clinic's multidisciplinary team of ancillary staff and providers who provide quality & safe patient care and by respecting patients privacy, and treating all patients, family members, and guests, as well as other associates, with dignity, respect, and compassion. * Evaluates patient prenatal course and adapts plan of care appropriately as part of multi-disciplinary team to ensure continuity of care and concerns. * Responsible for communication and outreach to establish and maintain relationships with physicians, floor and office nursing, radiology, laboratory, social services, support staff and any other personnel needed for patient care. * Documents assessments, interventions, progress and all communications with patients and clinic colleagues. |
