Overview

The Senior Quality Outcomes RN supports Flagstaff Medical Center and Verde Valley Medical Center to provide safe, effective, patient-centered, efficient and equitable care through continuous monitoring of key metrics and interdisciplinary collaboration. The Senior Quality Outcomes RN acts as an internal consultant to enhance value through quality improvement and innovation of systems. This position works directly with senior leaders, managers, clinicians, and other staff to develop and implement improvements in quality, safety, and efficiency. Supports the capabilities of managers, clinicians, and administrators by providing Evidence Based recommendations and Lean Six Sigma tools to accomplish specific objectives. Collaborates with the Director of Quality and the Quality Outcomes Manager to promote standard work processes within the Quality department and orchestrates team efforts to ensure that Quality RN products meet deadlines and expectations. Demonstrates mastery of Quality concepts and safety science principles and has experience applying those concepts and principles in a dynamic healthcare environment.

Matriculation into the Senior Quality Outcomes RN position requires completion of a portfolio outlining success in each essential job function and interviewing with the Chief Quality Officer, the Director of Quality, and the Quality Outcomes Manager to determine suitability for the position.

Responsibilities

Professional Engagement

Quality Leadership and Integration
* Advance healthcare quality throughout the organization through collaboration, learning opportunities, and communication.

* Implement processes that engage stakeholders and promote interprofessional teamwork.

* Communicate effectively with nursing staff, providers, patients, and leadership to achieve quality goals.

* Apply procedures to regulate the use of privileged or confidential information.

Quality Review and Accountability
* Promote compliance with voluntary, mandatory, and contractual reporting requirements for data acquisition, analysis, reporting, and improvement.

* Support peer-protected processes such as Ongoing Professional Practice Evaluation (OPPE), Mortality and Morbidity (M&M) committees, Clinical Outcomes Review Committee (CORC) review, and Infection Prevention (IP) review.

* Implement processes to facilitate practitioner performance review activities.

* Demonstrate knowledge of the impact of healthcare payment models as they apply to quality measurement and improvement programs.

* Work with Quality Outcomes RNs to develop executive summaries and detailed department level reports.

* Develop systems for tracking opportunities for improvement (OFIs) related to review outcomes and ensuring accountability.

* Analyze the appropriate utilization of healthcare services.

* Tie quality work to strategic goals or ratings from external agencies such as CMS, Leap Frog, and U.S. News and World Report.

Performance and Process Improvement
* Evaluate process improvement methods (e.g. LSS, PDSA) and apply those best suited to achieving successful outcomes.

* Use a range of quality improvement tools and techniques (e.g. 5 Whys, Value Stream Mapping, Failure Modes Effects Analysis, and Root Cause Analysis) and evaluate the output to successfully redesign processes toward improved performance.

* Access and use information resources (Electronic Health Records, registries, databases, comparative and benchmark data, and published research) to demonstrate current practice, identify opportunities for improvement, and strive toward improved outcomes.

* Apply project management methods to track progress on key quality initiatives and quality-led projects.

* Use change management principles and tools to plan projects and guide implementation.

Patient Safety
* Assess the organization's patient safety culture and work with leaders on identifying behaviors that impact safety culture.

* Apply safety science principles and methods to identify root causes and potential solutions when safety risks are discovered.

* Identify and report any safety related incident in a timely fashion through the Midas/RDE tool; attend all safety related training programs; perform work in a safe manner; monitor care environment for possible safety issues and ensure others are also performing work in a safe manner.

* Collaborate with organizational partners to analyze patient safety risks and events.

* Develop methods for sharing the results of patient safety work for organization-wide awareness and learning.

Health Data Analytics
* Leverage analytics to help guide data driven decision making and inform quality improvement initiatives.

* Design data collection plans for key metrics and performance indicators.

* Acquire data from source systems and integrate data from multiple systems to fully understand complex issues.

* Communicate results using narrative and visual tools (e.g., graphs, dashboards, and scorecards) to provide a comprehensive context appropriate to the audience.

* Apply common analytical and statistical tools and techniques used in quality and performance improvement work.

* Develop procedures for the governance of quality data assets and work with quality leadership to ensure consistent application of data governance procedures.

Regulatory and Accreditation
* Operationalize processes to support compliance with regulations and standards.

* Collaborate with organizational partners to ensure continuous survey readiness.

* Perform gap analyses of regulations and standards that involve quality and develop plans to achieve full compliance.

Population Health and Care Transitions
* Integrate population health management strategies into quality work.

* Apply a holistic approach to improvement.

* Collaborate with stakeholders to improve care processes and transitions.

Compliance/Safety
* Responsible for reporting any safety related incident in a timely fashion through the Midas/RDE tool; attends all safety related training programs; performs work in a safe manner; monitors work environment for possible safety issues and ensures others are also performing work in a safe manner.

* Stays current and complies with state and federal regulations/statutes and company policies that impact the employees area of responsibility.

* If required for position, ensures all certifications and/or licenses are up-to-date and valid prior to expiration dates.

* Completes all company mandatory modules and required job specific training in the specified time frame.

Qualifications

Education
 
BSN - Required

Masters in Nursing or Healthcare Related Field - Preferred
 
Certification & Licensures

Certification in Healthcare Quality, Patient Safety, or Process Improvement (e.g.. CPHQ, CPPS, ASQ Green Belt) - Required

Active, unrestricted, Arizona RN License or valid, participating compact license - Required
 
Experience
 
3 years clinical practice experience in adult acute care setting - Required

Experience as formal leader in local or system process improvement initiatives - Required

Experience mentoring and motivating colleagues to achieve practice excellence - Required

Experience networking with multiple healthcare professionals and identifying stakeholders when planning practice change - Required

Experience with data management and ability to interpret data and implement change based on factual information - Required
