The Lead Care Coordinator is a member of the multidisciplinary team that actively facilitates those functions that move a patient through an acute episode of care and links to internal and external resources and the payor community. The Lead Care Coordinator leads the process which identifies complex outlier patients and plans, implements, monitors, and measures the effectiveness of more complex interventions to meet patients' treatment and transitional needs. The Patient Safety Attendant under the supervision of a Registered Nurse, is responsible for providing generalized support services in accordance with the Nursing Plan of Care and Medical/Surgical Standards of Care and Practice. S/he must follow all policies/procedures of Northern Arizona Healthcare. S/he performs other duties, as assigned or directed, that are within the scope and abilities of the individual's training. The PSA may interact with patients and/or families, children, adolescents, adults, and geriatrics.
The PCT, including all hospital units and departments and the Float/Flex Pool PCT, under the supervision of the charge nurse, clinical manager, and/or director, is responsible for assisting in patient care, documentation and supply management. PCTs keep the unit supplied, clean, and organized. Additionally, the PCT may function in an office assistant role and answer the telephone, answer questions within their capabilities the patient or their support individuals may have, and perform data entry and data collection.
The PCT in the discharge unit is for assisting discharged patients, coordinating discharge logistics and assisting patients in utilizing resources and accessing health related services.
