The Manager Chaplaincy provides a support system to patients and families, acts as a spiritual counselor to staff members and serves as a spiritual liaison to the local community. The Medical Office Receptionist is able to function with minimal supervision and demonstrates appropriate decision making skills within established guidelines, maintains strict confidentiality, and utilizes discretion at all times. The PCT in the Labor Delivery and Post Partum Breastfeeding Center does not provide direct patient care.   The PCT follows all policies and procedures of Northern Arizona Healthcare. They perform or assist with procedures that fall within the scope of practice of a PCT, and work closely with all members of the interdisciplinary team. The PCT may be caring for infants, children, adolescents, adults and geriatrics populations.   The Float/Flex Pool PCT may be asked to work on any unit in the hospital that is in need of a PCT, change departments during a shift or be asked to work as a Patient Safety Attendant based on need whereupon the PCT duties will cease if assigned to sit 1:1 with a patient; and must complete the same required documentation as a PSA. Participates on committees and hospital rounding teams. The Case Management Assistant provides transition support for post-acute care needs, and preventative outpatient services in order to support appropriate delivery of care. He/She serve as an integrated team member to ensure patients achieve a safe and timely discharge from the acute care to the outpatient setting. Responsibilities center around support to the hospital and community-based service requests from the nursing staff, Care Coordinators, Call Center, Care Management, Therapists, Hospitalists, or private physician's practices in the PathfinderHealth ACO.
