The Patient Care Technician (PCT) Clinic, under the supervision of the Practice Supervisor or Manager, nurse, therapist, lead or Director, is responsible for assisting in patient care, documentation and supply management in the clinic or outpatient department. S/he may collaborate with the Medical Assistant to support the provider(s) in providing patient care.
The PCT Clinic will also work to keep the unit supplied, clean, and organized. The PCT Clinic may function in an office assistant role within the clinic and/or department and answers the telephone, answers questions within their capabilities the patient or their support individuals may have. S/he follows all policies and procedures of Northern Arizona Healthcare. They perform or assist with procedures that fall within the scope of practice of a PCT in addition to office assistant duties. The PCT Clinic may be caring for infants, children, adolescents, adults and geriatrics populations.
Under general supervision of the provider, the Medical Assistant (MA) Certified works with members of the departmental health care team to provide assistance to providers, patients, and their families ensuring smooth work and patient flow. The MA Certified, under the supervision of the provider, is responsible for conducting initial and on-going assessments of the patient and working with the provider on their procedures and/or exams. This includes but is not limited to, reviewing the patient medical record and conducting per-visit interview with patient, documenting medical history and assessing need for referrals and prescription renewals. The MA Certified also assists in the coordination of clerical information to provide patient care and assists with patient follow up.
In Community Care Management, the MA also assists in following up with health management/care plans with both patients and providers. The MA works closely with medical providers, primary care teams and other agencies to improve communication, services, patient care and outcomes.

The Lead Care Coordinator is a member of the multidisciplinary team that actively facilitates those functions that move a patient through an acute episode of care and links to internal and external resources and the payor community. The Lead Care Coordinator leads the process which identifies complex outlier patients and plans, implements, monitors, and measures the effectiveness of more complex interventions to meet patients' treatment and transitional needs. Requirements of the job may include support to Lead Utilization Review nurse and Utilization Review team. The Lead Care Coordinator acts as an advocate for the shift, making sure the Clinical Manager/Director are aware of the unique needs of the shift. leadership to the department, the Charge RN acts as an advocate for the shift, making sure the Clinical Manager/Director are aware of the unique needs of the shift. The Charge RN provides leadership for nursing personnel by being a team leader, serving as a preceptor,
and being an active member of the health care team. The Charge RN practices within the ANA Scope of Practice, the ANA Code of Ethics for Nurses and the standards of his/ or her specialty.
The Clinical Social Worker is responsible for participating in interdisciplinary care of patients and their families with complex medical and social needs. The Social Worker provides supportive counseling, crisis intervention, case management and community resource referrals. S/he functions as a member of a multi-disciplinary team to facilitate the delivery of services to patient and families. The social worker collaborates with the rest of the team to identify, assess, plan, implement, coordinate, monitor and evaluate options and services to meet the complex needs of patients.
This role lends emotional and social support during the active treatment and recovery period. This includes assisting patients in meeting their emotional, psychosocial needs, performing diagnostic assessments, crisis interventions, social and financial concerns, and coordinating a plan of care to address patient/family needs and providing information and referrals as needed. This position in the Palliative Care Clinic and Radiation Oncology Clinic also includes performing short-term individual and family psychotherapy.
This position facilitates support groups and education activities for patients, families and the general public. Provides case management services including referrals to homecare, hospice and supportive care with a multitude of community agencies and other agencies. Assists patients with housing and transportation needs.

Provides technical services as assigned and directed. The OR Scrub Tech assists and/or performs patient procedures that are within the scope of their unit specific Skills Summation. This position works in an in-patient / hospital based environment.
The Diagnostic/Xray Radiology Technologist performs diagnostic radiographic procedures using stationary and mobile radiographic equipment according to established standards. This position provides basic patient care, processes films, maintains equipment, transports patients and prepares and maintains documentation.
Diagnostic/Xray Technologist 1 works in the following environments predominately: outpatient, pain/palliative care, children's health, orthopedics, portable and inpatient, and may work in less than three of the following: fluro, o-arm, trauma, operating room and dexascan.
