The Grant Program Counselor assists patients and families in meeting their bio-psycho-social needs by means of support services and therapeutic techniques. These will include but will not be limited to assessment, consultation, teaching, support services for patients and families, crisis intervention, group psychotherapy, individual psychotherapy, family psychotherapy, interdisciplinary treatment planning and discharge planning. The Grant Program Counselor works in Safe Child Center as part of the child abuse intervention team.
Ensuring the achievement of Northern Arizona Healthcare’s mission, the Registered Nurse is accountable for the coordination of care for patients and their families including direct care, patient and family education and discharge planning. Under general supervision and reporting to the Manager of HIM, the Senior HIM Processor is responsible for processing release of medical information requests, review of charts for physician incomplete and missing documents according to Hospital Policy and Medical Staff Bylaws and Rules and Regulations, processing medical records, performing analysis on medical records and providing excellent customer service for requests.

The Clinical Dietitian works with the interdisciplinary healthcare team to ensure optimum care for both inpatients and outpatients by completing medical nutrition therapy assessments, instructions and care plans. The Clinical Dietitian in the Nutrition Services department will perform an annual competency evaluation and complete annual evaluations for the clinical aspects of all Dietitian positions. All patient menus, recipes and nutrient values will be placed in room serve choice and updated when changes are made, and reviewed annually.
The PCT, including all hospital units and departments and the Float/Flex Pool PCT, under the supervision of the charge nurse, clinical manager, and/or director, is responsible for assisting in patient care, documentation and supply management. PCTs keep the unit supplied, clean, and organized. Additionally, the PCT may function in an office assistant role and answer the telephone, answer questions within their capabilities the patient or their support individuals may have, and perform data entry and data collection.
The PCT in the discharge unit is for assisting discharged patients, coordinating discharge logistics and assisting patients in utilizing resources and accessing health related services.
Assists in providing ticket resolution, analysis, configurations, coordination, testing, and installation of technological equipment and mobile device based hardware, software, peripherals, local area networks (LANs), and other related end user devices equipment. May assist with informal training of computer and mobile device users. The Patient Advocate is responsible for maintaining contact with prospective Bariatric patients from the time of the Information Session through to surgery. S/he will maintain databases in readiness at all times for use for the Center of Excellence designation. The Patient Advocate will act as a liaison between patients, staff, and physicians to support excellent customer service.
