The Clinical Social Worker is responsible for participating in interdisciplinary care of patients and their families with complex medical and social needs. The Social Worker provides supportive counseling, crisis intervention, case management and community resource referrals. S/he functions as a member of a multi-disciplinary team to facilitate the delivery of services to patient and families. The social worker collaborates with the rest of the team to identify, assess, plan, implement, coordinate, monitor and evaluate options and services to meet the complex needs of patients.

We are looking for the right physician to join our collegial and supportive employed Hospitalist group at our Flagstaff, AZ campus at Northern Arizona Healthcare (NAH), playing a critical role in advancing our mission of "improving health, healing people". The hospitalist group is led by a working hospitalist, reporting to a physician. The NAH vision is grounded in our intention to transform the health of the communities we serve in Flagstaff, Sedona, Cottonwood and all of Northern Arizona. Flagstaff Medical Center is the transfer center, Level 1 trauma center, and tertiary care destination for Northern Arizona with a catchment that extends from past the New Mexico border to the California border, and brings challenging and interesting cases to our mid-sized hospital and city.
