Join our nutrition team at the Verde Valley Medical Center!   The Clinical Dietitian works with the interdisciplinary healthcare team to ensure optimum care for both inpatients and outpatients by completing medical nutrition therapy assessments, instructions and care plans. Provides presentations and learning opportunities for community.   The Clinical Dietitian in the Nutrition Services department will perform an annual competency evaluation and complete annual evaluations for the clinical aspects of all Dietitian positions. The Case Management Assistant provides transition support for post-acute care needs, and preventative outpatient services in order to support appropriate delivery of care. He/She serve as an integrated team member to ensure patients achieve a safe and timely discharge from the acute care to the outpatient setting. Responsibilities center around support to the hospital and community-based service requests from the nursing staff, Care Coordinators, Call Center, Care Management, Therapists, Hospitalists, or private physician's practices in the PathfinderHealth ACO.

The Medical Office Receptionist undertakes a variety of day-to-day office and clerical tasks to ensure that office operations run smoothly and are successful in supporting other business activities. The Medical Office Receptionist obtains insurance information, collects payments and works with the scheduling system as well as acts as an interdepartmental staff liaison between staff, providers and customers to ensure professional service and promote customer relations. The Medical Office Receptionist is able to function with minimal supervision and demonstrates appropriate decision making skills within established guidelines, maintains strict confidentiality, and utilizes discretion at all times. The Switchboard Operator/Dispatcher is responsible for all hospital switchboard/dispatch operations. The Operator/Dispatcher provides customer service by performing administrative telephone answering duties, code/alarm monitoring duties and dispatching security officers to both emergency and non-emergency calls. The Operator/Dispatcher utilizes appropriate multi-line telephone equipment/dispatching equipment to answer basic and/or routine calls, emergency calls, transfer calls and dispatch to answer basic and/or routine questions and route or transfer calls to appropriate destinations. The Operator/Dispatcher is responsible for receiving, coordinating, and disseminating critical information from and to multiple sources/resources within acceptable time frames.

The RN Transfer Center, under the supervision of the Director and/or Clinical Manager, is responsible for facilitating admissions/transfers to and within NAH. S/he maintains the Physician Referral Line service to provide timely access to area specialists as needed. S/he will direct, delegate, and coordinate the process needed for patient access NAH. S/he follows accepted nursing practice, policy, and procedures. S/he performs procedures that are within the scope of department and nursing practice. S/he seeks assistance and support when necessary, assists in data collection, and facilitates admissions or transfer of patients for patient care. Maintains accountability for own practice and those functions s/he delegates. Within a specified work period, plans and coordinates patient placement and physician consults.

This includes reviewing the patient medical record, completing and documenting vital signs, recording chief complaint, documenting medical history, assisting in referrals, prescription renewals and patient follow ups.
