The Community Care Coach (CCC) will primarily be working out in the community with specific target populations, teaching the necessary tools for patients to properly navigate the healthcare system, encouraging the adoption of habits that are conducive to a higher quality of life, and implementing programs designed to increase overall health. The CCC will work under the guidance of the RN or SW to conduct and document biometric screenings, help patients/clients in utilizing resources and accessing health related services, coach patients/clients in effective management of health conditions, and will assist in following up with health management/care plans with both patients and providers. The Patient Scheduling Representative is responsible for the verification and collection of patient demographic and insurance information by direct data entry to the electronic medical record during the scheduling/referrals. S/he conducts either face-to-face or inbound/outbound telephonic interviews with the patient or authorized representative to secure information specific to requested services; accurately documenting the discussion and other referral/scheduling activities in the encounter, schedule book, and patient chart.
Demonstrates customer-centric focus in all interactions with internal and external customers as well as an understanding of and ability to achieve acceptable performance standards as defined by Integrated Patient Scheduling Management.
The primary location of this position is hybrid (remote and on site as needed).
The Clinical Social Worker is responsible for participating in interdisciplinary care of patients and their families with complex medical and social needs. The Social Worker provides supportive counseling, crisis intervention, case management and community resource referrals. S/he functions as a member of a multi-disciplinary team to facilitate the delivery of services to patient and families. The social worker collaborates with the rest of the team to identify, assess, plan, implement, coordinate, monitor and evaluate options and services to meet the complex needs of patients.
This role lends emotional and social support during the active treatment and recovery period. This includes assisting patients in meeting their emotional, psychosocial needs, performing diagnostic assessments, crisis interventions, social and financial concerns, and coordinating a plan of care to address patient/family needs and providing information and referrals as needed. This position in the Palliative Care Clinic and Radiation Oncology Clinic also includes performing short-term individual and family psychotherapy.
This position facilitates support groups and education activities for patients, families and the general public. Provides case management services including referrals to homecare, hospice and supportive care with a multitude of community agencies and other agencies. Assists patients with housing and transportation needs.

The Laboratory Assistant is responsible for the clerical aspects of post analytical testing and interacting with others using a variety of communication tools. Must handle bio-hazardous and infectious material while following universal precautions and OSHA guidelines.
